If you are tired all the time, and sleeping more has not made much difference, here is the short version. Persistent fatigue almost always has a reason behind it. In India, a small number of causes account for most of it: low iron stores, vitamin B12 and vitamin D gaps, thyroid changes, blood sugar swings, sleep that is long enough but not restorative, and a stress load that never switches off.

Some of these show up on a basic blood panel. Others need a closer look at how your body makes and uses energy. This guide covers what different kinds of tiredness point toward, the causes worth investigating, the signs that need a doctor, and how to work out which one is yours instead of guessing.

What "always tired" actually means

Tiredness is not one experience. The pattern you have points toward different causes, so naming yours is the first useful step.

Eight common reasons behind low energy

1. Low iron stores, even without anaemia

Iron carries oxygen around your body. When stores run low, everything costs more effort. Anaemia is common in India, and national health survey data has repeatedly found it in more than half of women aged 15 to 49.

Less well known is that ferritin, your iron store marker, can be low while haemoglobin still reads normal. Many people are told their haemoglobin is fine and go home still exhausted.

Recognise it by: breathlessness on stairs, pale skin or inner eyelids, hair fall, restless legs at night, poor exercise recovery, heavy periods.

What can be measured: haemoglobin, ferritin, and where relevant, iron studies.

What tends to help: correcting a confirmed gap under medical guidance. Iron is not a supplement to take blind, because too much is a genuine problem.

2. Vitamin B12 and vitamin D gaps

B12 comes mainly from animal foods, so long term vegetarian and vegan diets carry a higher risk of running low. Vitamin D deficiency is widespread across India, including in sunny regions, because most working days are spent indoors.

Low levels of either are associated with tiredness, low mood, poor concentration and muscle aches. B12 in particular can produce a fatigue that feels neurological, a heaviness and fog rather than sleepiness.

What can be measured: serum B12, and 25-hydroxy vitamin D.

3. Thyroid function

Your thyroid sets your metabolic pace. When it runs slow, energy, mood, digestion, temperature and weight shift together. Hypothyroidism is common in India, more common in women, and often picked up late because the symptoms are easy to blame on a busy life.

Recognise it by: tiredness with weight gain despite no change in eating, feeling cold when others do not, dry skin, hair thinning, constipation, low mood, heavier periods.

What can be measured: TSH, and where a doctor advises, free T3, free T4 and thyroid antibodies.

What tends to help: this one is medical. If your thyroid is underactive, a doctor manages it, and lifestyle support sits alongside that rather than instead of it.

4. Blood sugar swings

A large lunch built mainly on rice or roti, with little protein or vegetables, produces a rise in blood sugar and then a fall. That fall is what the 3 PM slump usually is.

Over time, a pattern of swings and a body responding less well to insulin can leave you tired much of the day, hungry soon after eating and craving sugar at night. 5. Sleep that is long enough but not restorative

Hours in bed and sleep quality are different measurements. You can spend eight hours asleep and wake unrefreshed if the sleep was fragmented, if breathing was disrupted, or if alcohol shortened the deeper stages.

Snoring with pauses in breathing is one of the most under-recognised causes of daytime exhaustion in India, and it is assessed by a doctor, not by a wellness plan.

Recognise it by: waking unrefreshed most days, morning headaches, loud snoring, someone telling you that you stop breathing, needing caffeine before noon to function.

6. A stress load that never switches off

Sustained stress is expensive. It holds your nervous system in a state of readiness that consumes energy even when you are sitting still, and it usually damages sleep at the same time, so the debt compounds. The recognisable version is being exhausted and wired at once.

7. How your cells produce energy

Every cell contains mitochondria, the structures that convert food and oxygen into usable energy. They need specific nutrients for that work, including B vitamins, magnesium, CoQ10 and carnitine. When intake is poor, demand is high, or sleep and stress are chronically unfavourable, energy production tends to become less efficient.

When to see a doctor

Most fatigue is not sinister. Some is. Please see a doctor promptly if your tiredness comes with any of the following.

  • Unexplained weight loss
  • Breathlessness at rest or on mild exertion, chest pain, or a racing heart
  • Fever, night sweats, or a lump anywhere
  • Blood in your stool or urine, or unusually heavy menstrual bleeding
  • Severe daytime sleepiness, particularly falling asleep while driving
  • Loud snoring with pauses in breathing, or waking gasping
  • Persistent low mood, loss of interest or hopelessness lasting more than two weeks
  • Fatigue lasting more than four weeks without an obvious explanation
  • New confusion, weakness on one side, or difficulty speaking. This is an emergency

How to find out which cause applies to you

Here is the honest problem with fatigue advice, including everything above. Every guide lists causes. None can tell you which one is yours. So most people change six things at once, cannot tell what helped, and conclude that nothing works.

Measurement replaces guessing, and it does not have to be expensive.

Start with blood markers. For persistent tiredness the useful panel usually includes haemoglobin and ferritin, vitamin B12, vitamin D, TSH, fasting glucose and HbA1c. Between them these cover most of the physical causes. Frequently asked questions

Why am I always tired even after eight hours of sleep? Hours and quality are different things. Sleep that is fragmented, disrupted by breathing pauses, or shortened in its deeper stages by alcohol leaves you unrefreshed regardless of time in bed. Which blood tests should I ask for if I am always tired? A useful starting panel is haemoglobin with ferritin, vitamin B12, vitamin D, TSH, fasting glucose and HbA1c. Can low iron cause tiredness without anaemia? Yes. Ferritin measures your iron stores, and it can fall well before haemoglobin does. Many people are told their haemoglobin is normal while stores are already depleted, which is why tiredness, hair fall, poor recovery and restless legs appear without a formal anaemia result. Ask for ferritin specifically, and never start iron supplements without testing.

Why do I crash at 3 or 4 in the afternoon? Usually the composition of lunch. A meal built mainly on refined carbohydrates raises blood sugar quickly, and the fall that follows shows up as heaviness and poor concentration. Caffeine timing, dehydration, poor sleep and sitting still all deepen it. Adding protein and vegetables to lunch, then walking for 10 minutes, helps many people within days.

Could my tiredness be thyroid related? It could be, particularly alongside feeling cold, dry skin, hair thinning, constipation, low mood or weight gain without a change in eating. Can vegetarians get enough vitamin B12? It is harder, because B12 comes mainly from animal foods.
