Personalized Supplements: How To Know What You Actually Need

Open most supplement cabinets in India and you will find two things at once: bottles the owner does not need, and no sign of the one or two things they might genuinely be short of.

That is not carelessness. It is the predictable result of buying supplements the way almost everyone buys them, on the strength of an advertisement, a friend's suggestion or an article written for nobody in particular.

The answer is not a longer shelf. It is knowing which items apply to you, which is a question with a real answer. Your blood markers, your genetics, your gut and your diet between them settle most of it.

This guide explains what actually determines your needs, where Indian diets tend to fall short, and how to work it out in a sensible order.

Why most supplement routines are guesswork

Ask someone why they take a particular capsule and the answer is usually a story about somebody else. A reel said it helps with energy. A colleague swears by it. An article recommended it for people over 35.

None of those sources knows your vitamin D level, whether you absorb B12 well, what you eat on a normal Tuesday, or what medicines you take.

Four consequences follow, and most people experience at least two.

Money goes nowhere. Five products at ₹700 to ₹1,500 a month comes to roughly ₹42,000 to ₹90,000 a year. A blood panel that would tell you which of them is justified costs ₹2,500 to ₹5,000 once.

Nothing changes, so you conclude supplements do not work. A supplement corrects a shortfall. If the shortfall was never there, there is nothing for it to do.

The gap you actually have stays unaddressed, because it was never on the list you copied.

Occasionally there is harm. Iron taken on assumption, very high doses of fat-soluble vitamins, or a herbal product interacting with a prescription are genuine risks, not theoretical ones.

Here is the part most supplement pages will not print. A person eating a varied diet, sleeping properly and getting some sunlight may need nothing at all. That is a good outcome, and if it applies to you, the honest recommendation is to spend the money on better food.

What actually determines what you need

Seven inputs. Together they explain almost every difference between two people standing in the same pharmacy aisle.

Your genetics

Folate processing, vitamin D receptor activity, vitamin B12 handling, the conversion of plant omega-3 into the forms your body uses, and caffeine clearance all vary between individuals.

This is stable information. You learn it once and it stays relevant for life, which is a different kind of value from a test you repeat every year.

Your gut, because absorption matters as much as intake

What you swallow and what reaches your bloodstream are not the same number. Gut inflammation, low stomach acid, coeliac disease, recent antibiotics and an imbalanced microbiome all reduce absorption.

Someone with a real absorption problem can take a sensible amount and still show a low result, which is why retesting after a few months matters as much as testing before.

Your blood markers

This is the only input that shows where you are right now. Vitamin D, vitamin B12, ferritin with a complete blood count, thyroid function and HbA1c cover most of the reasons people start supplements at all.

Genetics tells you what you tend toward. Blood markers tell you where you actually are. They frequently disagree, and the disagreement is informative.

Your diet

A predominantly vegetarian Indian diet has different and fairly predictable gaps from a diet including fish and eggs. A diet built on refined flour differs from one built on millets, dal and vegetables.

Your life stage

Adolescence, pregnancy and breastfeeding, the postnatal period, perimenopause and menopause, heavy training loads and older age each shift what matters. Pregnancy in particular is managed by your obstetrician, not by a website.

Your medication

Interactions are real and easy to miss. Blood thinners, thyroid tablets, certain antibiotics, acidity medicines and antidepressants all have known interactions with common supplements. Anything you add belongs in a conversation with the doctor or pharmacist who knows your prescriptions.

Your symptoms and goals

Wanting steadier afternoon energy is a different problem from correcting a confirmed low ferritin result. The first is a goal, the second is a gap. Both are legitimate, and mixing them up is why so many routines grow without ever being reviewed.


The gaps most common in Indian diets

Population data does not tell you about yourself, but it does tell you where to look first.

The ICMR National Institute of Nutrition publishes dietary guidelines written for Indian kitchens, which are more useful here than most imported advice. On iron, the National Family Health Survey (NFHS-5) reported anaemia in roughly 57 percent of Indian women aged 15 to 49, which is the strongest reason not to guess in either direction on this one.

What is usually already fine. Vitamin C, if amla, guava, citrus or capsicum appear regularly. Most B vitamins other than B12, in a mixed diet with dal, curd and whole grains. That matters, because these are among the most commonly bought supplements in the country.

Protein is a food gap rather than a supplement gap in most Indian households, and it is cheaper to fix with dal, curd, eggs, paneer and chana than with a tub of powder.


When to speak to a doctor

Some symptoms need a doctor, not a shopping decision. Please see a qualified doctor rather than buying something if you have:

  • Fatigue with breathlessness, palpitations or unusual paleness
  • Heavy or prolonged menstrual bleeding
  • Unexplained weight loss
  • Numbness, tingling, or problems with balance
  • Blood in your stool, or a change in bowel habit lasting weeks
  • Persistent low mood, anxiety or loss of interest lasting more than two weeks

That last point deserves emphasis. If low mood is the reason you are looking at supplements, please speak to a doctor or a qualified mental health professional. That support matters more than anything you can buy.

Some people must ask before starting anything at all. Speak to your doctor first if you are pregnant or breastfeeding, are under 18, take any prescribed medication, live with kidney, liver, thyroid or autoimmune conditions, or have surgery planned. Take the actual bottles with you, including herbal products, since those are the ones most often left out of the conversation.

Never stop, reduce or replace a prescribed medicine because of something a supplement label or seller says.

One general point on doses, which this page deliberately does not give. More is not better. Iron and the fat-soluble vitamins A, D, E and K carry real risks at high intake, and the NIH Office of Dietary Supplements publishes the upper limits your doctor works from.


How to find out what you need, in order

The order is what saves you money. Most people run it backwards, buying first and wondering later.

1. Start with a blood panel. Vitamin D, vitamin B12, ferritin with a complete blood count, thyroid function and HbA1c are a sensible starting set for most adults. This is the only step that shows your current status, and in India it is inexpensive next to a year of supplements. Add whatever your doctor suggests for your history.

2. Add genetic insight. Blood markers show today. Genetics shows tendency, and explains why two people eating similar food end up in different places. 4. Review your diet honestly. Write down a normal week. If protein is missing at most meals, if vegetables appear rarely, or if you never see daylight, fixing that will do more than anything on a shelf.

5. Build a routine, then set a review date. Correct only the confirmed gaps, one change at a time, then hold for eight to twelve weeks before retesting. What to look for in any supplement, whoever makes it

Use this on every brand you consider, including ours.

  • Quantities disclosed per ingredient. If a label gives one combined weight for eight ingredients under a proprietary blend, you cannot know what you are getting.
  • Amount per serving, and what a serving is. Many products only reach their headline number across three or four capsules a day, and most people take one.
  • A form your body absorbs well. This matters most for iron, magnesium, vitamin D and vitamin B12, where the cheapest form is often the least useful.
  • Third party testing, with a certificate of analysis available. Heavy metal contamination is a genuine issue in parts of the herbal supplement market.
  • A valid FSSAI licence number on the pack. Nutraceuticals sold in India come under FSSAI rules.
  • Realistic claims. A product promising a specific outcome by a specific date, or describing itself as a replacement for medical care, is telling you something useful about the company.
  • A reason for you specifically. The best formulated product in the world is wasted money if you do not have the gap it addresses.

Five things you can do this week without buying anything

  1. Empty the cabinet onto a table. Check expiry dates, then check whether you can say why you started each one. Anything with no answer goes into a pause and review box, not the bin.
  2. Book the blood panel instead of the next bottle. One test replaces a year of guessing, and it costs less than two months of a typical routine.
  3. Get 15 to 20 minutes of direct daylight on your arms most days. It is free, and it is relevant to the most common shortfall in the country.
  4. Add protein to breakfast. Curd, eggs, chana, paneer, sprouts or two idlis with sambar. This closes more real gaps than most capsules.
  5. Write down what you expect to notice, before you start anything. "Fewer afternoon crashes" is testable. "Feeling better" is not, and untestable goals are how shelves grow.

How do I know which supplements I actually need? Do I need a multivitamin? Many people do not. In healthy, well nourished adults, research on routine multivitamin use has generally found little measurable benefit for major health outcomes. A multivitamin also tends to give small amounts of many things rather than a useful amount of the one or two you may actually be short of. Testing first is the more sensible route.

Can supplements be personalised based on DNA? Partly. Genetic analysis can identify factors associated with how you handle vitamin D, folate and other B vitamins, and omega-3 fats, which helps explain differing requirements. It cannot tell you your current level of anything, so it works best alongside blood markers rather than instead of them. Be cautious with any recommendation based on DNA alone.

Should I get a blood test before starting supplements? For most people, yes. A blood panel is the only way to know your current status, and it gives you a starting number so you can tell later whether anything changed. It typically costs a fraction of a year of supplements. Discuss which markers suit your history with your doctor.

Can I take multiple supplements together? Some combinations help, some interfere. Iron and calcium compete for absorption, and tea or coffee with an iron supplement reduces uptake considerably. Fat-soluble vitamins absorb better with food containing fat. Add one item at a time, roughly a week apart, so that you know what caused any effect. Our timing guide covers the common pairings.

Are supplements safe with prescribed medication? Not automatically. Known interactions exist with blood thinners, thyroid tablets, certain antibiotics, acidity medicines and antidepressants, among others. Always ask your doctor or pharmacist before adding anything, and take the actual bottles with you, including herbal products. Never stop or change a prescribed medicine on your own.

How long before I notice a difference? It depends on the nutrient and on how low you were. Most nutrient markers need roughly eight to twelve weeks to move meaningfully, so that is the sensible interval before retesting. Some people notice changes in energy or sleep sooner, some notice nothing at all, and results vary from person to person.

Are there supplements Indians commonly need more of? Low vitamin D levels are reported widely across the country, including in sunny regions. Vitamin B12 is a common shortfall on predominantly vegetarian diets, and iron matters particularly for women of reproductive age. Omega-3 intake tends to be low where fish is rarely eaten. These are population patterns and starting points for testing, not a shopping list.
